LUMIMEDS MEMBERSHIP TERMS AND CONDITIONS

These Membership Terms and Conditions ("Terms") apply to your enrollment in and use of LumiMeds' membership-based treatment plans. By enrolling in a LumiMeds membership plan, you agree to be bound by these Terms along with our Terms of Use and Privacy Policy.

1. Scope of Membership Services

LumiMeds membership plans provide facilitated access to affiliated, independent licensed healthcare providers for telehealth consultations, and coordination of prescription fulfillment through third-party pharmacy partners. Your membership may include:

  • Access to licensed healthcare providers for telehealth consultations and ongoing care
  • Prescription evaluation and fulfillment for compounded medications through licensed pharmacy partners
  • Monthly follow-up appointments with a licensed medical professional and, where applicable, a registered dietitian
  • Health tracking and progress monitoring through the LumiMeds platform
  • Medication delivery to your designated address nationwide

2. Eligibility Requirements

To enroll in a LumiMeds membership plan, you must meet the following criteria:

  • You are at least eighteen (18) years of age or legally emancipated
  • You are a resident of the United States
  • You have an active LumiMeds account with accurate, complete, and up-to-date information
  • You are not disqualified from receiving treatment based on the results of a medical screening conducted by a licensed provider
  • You reside in a state where LumiMeds and its pharmacy partners are authorized to operate

3. Payment Terms and Billing Authorization

By enrolling in a LumiMeds membership plan, you authorize LumiMeds to charge the payment method on file for recurring membership fees. Pricing, plan duration, and renewal terms are disclosed at the time of enrollment and may be modified with prior notice.

  • Commitment Term and Financial Obligation: Membership fees are charged at the beginning of each billing cycle. You are financially responsible for all charges incurred during your membership.
  • Pricing Transparency: All prices are fixed regardless of dosage. There are no hidden fees or surprises.
  • Payment Method: You are responsible for maintaining a valid payment method on file. Failed payments may result in suspension of services until the outstanding balance is resolved.
  • No Insurance Billing: LumiMeds does not accept or bill health insurance. All membership fees are the responsibility of the member.

4. Cancellation and Termination

Cancellation by the Member. You may cancel your membership at any time by contacting LumiMeds support or through your account settings. Cancellation will take effect at the end of your current billing period. No refunds will be issued for services already rendered during the current billing cycle.

Termination by LumiMeds.LumiMeds reserves the right to suspend or terminate your membership for any violation of these Terms, for conduct that is harmful to other members or providers, or for any other reason at LumiMeds' sole discretion.

Refunds. Refunds are provided when medically appropriate and medication has not been prescribed or shipped. Requests for refunds should be directed to help@lumimeds.com.

5. General Provisions

  • No Insurance Coverage: LumiMeds membership fees and services are not covered by health insurance plans.
  • Pharmacy Partners: LumiMeds is not a pharmacy and does not dispense medications. All medications are dispensed by licensed, independent pharmacy partners.
  • Compounded Medications: Compounded medications offered through LumiMeds are not FDA-approved and are not evaluated for safety or efficacy by the FDA.
  • Entire Agreement: These Terms, together with the Terms of Use and Privacy Policy, constitute the entire agreement between you and LumiMeds regarding your membership.
  • Amendments: LumiMeds reserves the right to amend these Terms at any time. Continued use of the service after amendments are posted constitutes acceptance of the revised Terms.
  • Governing Law: These Terms shall be governed by and construed in accordance with the laws of the State of Nevada.
